Will you be using insurance or self-pay for your visit?

Medicare clients: Please note that Medicare ONLY covers nutrition counseling for Diabetes or Chronic Kidney Disease. They do not cover any other diagnosis. If you have a Medicare Advantage Plan or a supplement plan for Medicare, they will still likely not cover nutrition counseling outside of those two conditions.

If Medicare is your main insurance and you also have a secondary plan like BCBS Federal — rather than a Medicare Advantage or supplement plan — your visits may be covered. Questions? Contact Érica at 406-510-8700 or erica@nutritionfocusedhealth.com.

I understand This doesn't apply to me

MT Medicaid clients: Medicaid ONLY covers nutrition counseling for those who are 20 years old or younger. If you are 21 years old or older, Medicaid will NOT cover nutrition counseling. If you are an adult with Medicaid and have Diabetes or Chronic Kidney Disease, I can refer you to a different clinic where you will have coverage.
